How can Digital Transformation solutions have a positive impact on better patient outcomes as they transition from their hospital stay to home care? With governmental pressures driving the trend towards greater accountability in healthcare, hospitals need to find ways to ensure that patients stay on their prescribed regimen of medications after they are discharged. What tools/solutions can be given to providers, ambulatory pharmacies and patients to ensure they are taking their medication consistently and appropriately and following correct procedures post-discharge. I am currently not part of any group, however, this concept sounds interesting. A proposed solution could be to create an application where providers or ambulatory pharmacies set up accounts for patients about to transition to home care that will help them keep track of their post-discharge lifestyle. Providers will be able to update the patient’s profile with their medication and daily regimen. The patient will then be able to download the app and receive notifications when it’s time to take their medication along with instructions on how to take it properly. Patients will also receive reminders to refill their prescriptions.
Upon opening the app, patients will be able to view their current prescription, instructions for taking their medication, doses left until a refill is needed, expected recovery time (if it is not a life-long plan), and any special care instructions or tips for recovery from the provider. A chat service could also be included that will allow patients to better communicate with their providers if they have questions about their prescriptions or if they are experiencing any problems. A web version could also be available for those without smartphones. The web version could sync with Google Calendars for the notifications and reminders.
The provider’s account could have a record of all home care patients. They will be able to view each patient’s profile, which will display the same information as provided on the patient’s account. There could also be an option to send patients messages in order to check up on them.
This is a nicely articulated idea. However, it one of what could be many solutions. And if you step back further, it could also be one of many interpretations of what the ‘real problems’ are. So this a good start and the value of working with an experienced executive is that they can provide a sounding board for these ideas. Get a team together! 🙂
