I have not had any of these happen to me. However, I believe number three, “Start working on the database without doing a full backup first”, is the most important. What struck me the most in terms of bias and the Signal Problem was that there is no overall cure for bias. No matter what anyone does to make a sample better (i.e., have it be random, have a bigger sample, etc.) bias will always be present. There is no full proof method to fully eliminate bias.
